What is the #PreventDiabetes program?
The #PreventDiabetes program is a 12-month program that provides tools to help you lose weight, reduce your risk for type 2 diabetes, and build healthy habits that will last a lifetime.
Members receive the following:
- 365 days of support with meal ideas, an exercise plan with workout videos and guides, daily tips, guided meditations, and more
- 26 educational on-demand video webinars
- Health Chat to message with others on your Success Team
- Weekly check-ins with a Lifestyle Coach
- Tracking tools for physical activity, food, goal setting, and weight
- Healthy selfie progress tracking with photographed weigh-ins on a HEALTHspot Scale
- Quarterly cash rewards based on weight loss and maintenance
- Personal dashboard accessed online or through the incentaHEALTH app
Through weighing in with an incentaHEALTH HEALTHspot scale at home or at a community kiosk, members can visually track their progress. Quarterly weight improvement enables participants to earn cash rewards.
The #PreventDiabets program is a virtual National Diabetes Prevention Program that follows the Centers for Disease Control and Prevention (CDC) Prevent T2 curriculum and has achieved full recognition status from the CDC.

Who can participate in the program?
The #PreventDiabetes program is available for members age 18 and over that meet the enrollment requirements to be at-risk for type 2 diabetes, including: having a BMI of 25 or greater, or based on A1C or glucose levels. How do I enroll?

- What should I expect once I enroll?
Daily Coaching: Receive a daily email, text message, or access through the incentaHEALTH app tips on H.E.A.L. (Healthy Eating and Active Living) principles. Messages are tailored based on enrollment preferences.
Complete Weekly Activities: Each week, members have three weekly activities to complete:
- Weekly Weigh-In
- Log Physical Activity Time (Goal > 150 minutes/week, but anything counts!)
- Watch On-Demand Webinar
Lifestyle Coach Support: Access to a certified Lifestyle Coach for questions or concerns regarding personal progress and for supporting your goals to prevent type 2 diabetes.
Optional Coaching Tools: On your #PreventDiabetes dashboard, you will find additional optional coaching tools including a food log, action plan journal, health quizzes, and an activity tracker function.
Measure: Weekly weigh-ins are required through the use of a kiosk, smart scale, or logged manually on the online dashboard. Members who utilize the kiosks or Smart Scale will be eligible for cash incentives based on weight improvement every 90 days. Please note that if you decide to submit a manual weigh in you will not be eligible for cash rewards based on weight improvement.
Reward: Eligible participants have the opportunity to earn quarterly cash rewards for reaching specified weight improvement marks (see Reward section for more details).

What are the required weekly activities?
Each week, members have three weekly activities to complete:
- Weekly Weigh-In - To be completed using an incentaHEALTH Smart Scale at home, a kiosk in the community, or manually entered into the Weekly Weigh-In tool on the #PreventDiabetes dashboard or app.
*Please note that manually entered weigh-ins are not eligible for quarterly cash incentives.
- Log Physical Activity Time (Goal > 150 minutes/week) - Active minutes must be recorded each week. This tool is broken down into individual days and allow for manual selection of each physical activity performed. Once saved, the week’s minutes are recorded.
- Watch On-Demand Webinar - Members are expected to view the available webinar which focuses on a new health topic. Allow one hour to complete webinar and corresponding activities. These education webinars reduce in frequency as you progress through the program.

What if I forgot my username and/or password?
Click the reset username or reset password link in the app. You can also contact a Health Coach at 800-694-0352(Monday-Friday, 8:00am-5:00pm) and we can confirm your password with you this way. For security reasons, we cannot provide your password to you via email. Remember, our password is case sensitive.

What happens after I have completed a year in the program?
Once you’ve successfully completed a year in the #PreventDiabetes program, you have the option to continue to complete quarterly weigh-ins, receive rewards, utilize the coaching tools, and continue on your personal health journey. Details of that option will be provided upon completion of your program. If you wish to end your participation, please let us know via phone or email.
