  1. What is the #PreventDiabetes program?

    The #PreventDiabetes program is a 12-month program that provides tools to help you lose weight, reduce your risk for type 2 diabetes, and build healthy habits that will last a lifetime.

    Members receive the following:

    • 365 days of support with meal ideas, an exercise plan with workout videos and guides, daily tips, guided meditations, and more
    • 26 educational on-demand video webinars
    • Health Chat to message with others on your Success Team
    • Weekly check-ins with a Lifestyle Coach
    • Tracking tools for physical activity, food, goal setting, and weight
    • Healthy selfie progress tracking with photographed weigh-ins on a HEALTHspot Scale
    • Quarterly cash rewards based on weight loss and maintenance
    • Personal dashboard accessed online or through the incentaHEALTH app

    Through weighing in with an incentaHEALTH HEALTHspot scale at home or at a community kiosk, members can visually track their progress. Quarterly weight improvement enables participants to earn cash rewards.

    The #PreventDiabets program is a virtual National Diabetes Prevention Program that follows the Centers for Disease Control and Prevention (CDC) Prevent T2 curriculum and has achieved full recognition status from the CDC.

  2. Who can participate in the program?

    The #PreventDiabetes program is available for members age 18 and over that meet the enrollment requirements to be at-risk for type 2 diabetes, including: having a BMI of 25 or greater, or based on A1C or glucose levels.   3. How do I enroll?

  4. What should I expect once I enroll?

    Daily Coaching: Receive a daily email, text message, or access through the incentaHEALTH app tips on H.E.A.L. (Healthy Eating and Active Living) principles. Messages are tailored based on enrollment preferences.

    Complete Weekly Activities: Each week, members have three weekly activities to complete:

    • Weekly Weigh-In
    • Log Physical Activity Time (Goal > 150 minutes/week, but anything counts!)
    • Watch On-Demand Webinar

    Lifestyle Coach Support: Access to a certified Lifestyle Coach for questions or concerns regarding personal progress and for supporting your goals to prevent type 2 diabetes.

    Optional Coaching Tools: On your #PreventDiabetes dashboard, you will find additional optional coaching tools including a food log, action plan journal, health quizzes, and an activity tracker function.

    Measure: Weekly weigh-ins are required through the use of a kiosk, smart scale, or logged manually on the online dashboard. Members who utilize the kiosks or Smart Scale will be eligible for cash incentives based on weight improvement every 90 days. Please note that if you decide to submit a manual weigh in you will not be eligible for cash rewards based on weight improvement.

    Reward: Eligible participants have the opportunity to earn quarterly cash rewards for reaching specified weight improvement marks (see Reward section for more details).

  5. What are the required weekly activities?

    Each week, members have three weekly activities to complete:

    • Weekly Weigh-In - To be completed using an incentaHEALTH Smart Scale at home, a kiosk in the community, or manually entered into the Weekly Weigh-In tool on the #PreventDiabetes dashboard or app.
      *Please note that manually entered weigh-ins are not eligible for quarterly cash incentives.
    • Log Physical Activity Time (Goal > 150 minutes/week) - Active minutes must be recorded each week. This tool is broken down into individual days and allow for manual selection of each physical activity performed. Once saved, the week’s minutes are recorded.
    • Watch On-Demand Webinar - Members are expected to view the available webinar which focuses on a new health topic. Allow one hour to complete webinar and corresponding activities. These education webinars reduce in frequency as you progress through the program.

  6. What if I forgot my username and/or password?

    Click the reset username or reset password link in the app. You can also contact a Health Coach at 800-694-0352(Monday-Friday, 8:00am-5:00pm) and we can confirm your password with you this way. For security reasons, we cannot provide your password to you via email. Remember, our password is case sensitive.

  7. What happens after I have completed a year in the program?

    Once you’ve successfully completed a year in the #PreventDiabetes program, you have the option to continue to complete quarterly weigh-ins, receive rewards, utilize the coaching tools, and continue on your personal health journey. Details of that option will be provided upon completion of your program. If you wish to end your participation, please let us know via phone or email. WEIGHING-IN

  1. How do I track my progress?

    Weekly weigh-ins are required through the use of a community kiosk, Smart Scale, or logged manually on the online dashboard.

  2. What types of weigh-ins qualify for cash rewards?

    Members who utilize the kiosks or Smart Scale will be eligible for cash rewards based on weight improvement. You are eligible for rewards every 90 days based on weight improvement visually tracked via a kiosk or Smart Scale.

  3. How does the Smart Scale work?

    The incentaHEALTH Smart Scale allows you to visually track your progress at home, and earn rewards!

  4. How to Weigh In With a Smart Scale:
    • Log in to your account in the incentaHEALTH app
    • Click the Weigh-In button
    • Bluetooth must be enabled on your phone; no pairing is necessary. You may also be prompted to allow camera access.
    • Position the phone so you take a full-length photograph (face and feet)
    • Step on the scale and smile! The Smart Scale must be placed on a hard surface (no carpet) to get an accurate weight reading.
  5. What is your warranty policy?

    We warrant the original purchaser that your incentaHEALTH Smart Scale shall be free from defects in materials and workmanship under normal use for a period of one year from the date of purchase.   6. Are my weigh-ins private?

    Your healthy selfies and weight measurements are saved on your own private dashboard and not shared externally. You have the option to share your weigh-in photographs and data to social media. A Health Coach will audit and view all weigh-ins prior to generating cash rewards to ensure members have proper weight readings and did not artificially influence their weight to create an inaccurate weight reading as per our User Agreement.

  7. What happens if I miss a quarterly weigh-in?

    If you miss a weekly weigh-in at a kiosk or Smart Scale, you have the option to manually enter your weight using the online weigh-in entry log. Please update this log or weigh-in at a kiosk or on a Smart Scale regularly to ensure your information is up-to-date and to see your personal progress.

    If you don’t weigh-in at a kiosk or Smart Scale before a weigh-in deadline, you will simply not be eligible to receive a reward that quarter. Since you have the entire three-month quarter to complete a visual weigh-in, your weigh-in deadlines are not flexible and no extensions will be offered.

    Your status in the program will not change in any way and you will not miss out on any rewards in future quarters. Your remaining weigh-in deadline dates will remain the same and if you are eligible for a cash reward based on your weight improvement, you will still be able to weigh-in during the following quarters to qualify for rewards.

REWARDS

  1. How much money is earned?

    Cash rewards are distributed quarterly and will be awarded to members who achieve weight improvement according to the below structure:

  2. How do I receive my reward?

    Rewards are distributed quarterly, based on your individual start date in the program. You can view your weigh-in calendar on your account. Members who earn cash rewards will receive instructions on how to claim their reward by email, app, or text. You can also access this information on your account. There is the option to receive a virtual or physical card. If/when rewards are earned in subsequent quarters, a new digital reward will be generated and you will be notified accordingly.

  3. How do I claim my reward?

    Your reward card link and PIN can be found on your account and in the notification email. Click the unique reward card link from your email or the ‘Manage Rewards’ button on your account. You will be prompted to enter your PIN. Click on the link displayed under “Redemption Instructions” to select your reward. This is where you can choose between a digital or physical reward card. A reward must be claimed in 90 days. All rewards expire within 6 months of being generated. A video of this process can be accessed here.

  4. How can I use my digital reward?

    You will be given a reward card number with a CVV and expiration date. You can manually enter this card for any online purchases or print off and bring it to a physical store where they can manually enter your card information.

  5. If I choose a physical card, how long will it take to arrive?

    Your physical card, if selected as preference, should arrive in the mail in 7-14 business days. You will receive a confirmation email from our card vendor once your card has been mailed out.

  6. What if I lose my physical card?

    Call Online Rewards at 1-877-325-8444. A representative is available from 8am-6pm CT.

  7. How do I check my card balance and transactions? Where do I find my card number?

    Click on your unique reward card link from your email, or the ‘Manage Rewards’ button on your Weigh and Win account. You will be prompted to enter your PIN. After entering your PIN, you’ll be sent a verification code to the email you provided when you selected your reward preference. After entering the verification code, you’ll be able to view your card information, card balance, and transactions.

  8. When do my rewards expire?

    A reward must be claimed in 90 days. All rewards expire after 6 months from issue date.

  9. Is there a way to combine card balances?

    There is no way to combine your rewards. A new digital reward will be generated every quarter you earn a reward. You can create a Digital Wallet to have all your rewards and balances available in one place.

  10. How long can people earn money?

    If weight improvement has been achieved and your starting BMI is 25 or greater, a member is eligible to receive a cash reward based on the amount of weight improvement. If weight is kept off, cash rewards can be received quarterly up to one year after program enrollment. If some or all of the weight is regained between weigh-ins, the cash reward will be reduced or eliminated in line with the reward structure goals. At the end of 12 months, the member’s baseline weight is recalculated and the incentive structure starts over with the new baseline weight for the program year. If the member’s starting BMI is still greater than or equal to 25, the member may still receive rewards for another year in the program for achieving weight improvement.

  11. What are HEALTHpoints?

    You can earn HEALTHpoints by engaging in the program. Tasks like weighing-in, opening your emails, and syncing an activity tracker will earn you points. Look under the HEALTHpoints section in the app or online to see what activities will earn you points and what prize drawings your subscription offers.
